ALL FAMILIES – Parent Portal Contact Info DUE

**Please note that all student records must be complete in order for your student to attend school. For returning students, most will just need an updated Record of Physical and a quick review of all other health information and contacts. 

Create an account by going to the Parent Portal page on our website. Your account is identified by your email address, so two parents can each create an account – both accounts will be able to access their child’s information – or, if you prefer, you can just create one account for both parents. When you create your account, you will receive a verification email – if you do not receive it, check your spam folder. Be aware that the verification code in the email will only work for approximately 30 minutes. Note that you can only create an account using an email address that Oak Meadow already has on file for you – if the email address that you want to use is not recognized, please contact Sarah Falcone.

Once you create your account, please log in and verify/enter the medical and emergency information as follows:

• Click on Profile and verify or complete the parent and household information

• For each child enrolled in the 2025-26 school year, click on their name and then Profile; complete all items, including:

  • Upload a recent physical, including immunizations

  • Specify 2 emergency contacts and up to 3 people authorized to pick up your child

  • Specify all permissions, including administration of OTC medications and photo permissions

• Be sure to click Submit at the bottom of each Profile

Tip: if you click “Change All” at the top of your child’s Profile, all items below will be unlocked, so you won’t have to click each individual Change button.
